This form should be completed by the person responsible for paying for lessons. Once submitted, we will email you confirmation of your schedule and payment will be taken on the first working day of each calendar month for lessons in the previous calendar month.

- Payment will be taken on the first working day of each calendar month for lessons in the previous calendar month.
- You agree to give us at least 48 hours' notice of any changes or cancellation of lessons. We will have to charge the full cost of the lesson if less notice is given, regardless of the reason.
